A referral may come from a patient's attending
physician; primary care physician; insurance company representative;
or hospital discharge planner (e.g. social worker, nurse or
case manager).
Following a referral for rehabilitation, a social worker or
discharge planner will coordinate the referral process. Every
referral is carefully considered on an individual basis.
Patients who need intensive rehabilitation should be referred
as soon as they are medically stable and able to participate
in an active rehabilitation program. Applicants for admission
should demonstrate the potential to reach rehabilitation goals
within a reasonable time period.
Applications can be forwarded by mail or, for maximum efficiency,
healthcare professionals may make direct referrals through

Families are encouraged to visit us for a tour of the hospital
and an explanation of the rehabilitation process. Individual
tours may be arranged by calling (478) ???-????
We accept Medicare, Medicaid as well as private insurance
and managed care reimbursement. Patients or their families
may contact the Business Office to make inquiries regarding
insurance coverage or billing. The office is open weekdays
during regular business hours.
