OUTPATIENT REFERRALS

Referrals for outpatient services do not require evaluation from Central Georgia Rehabilitation Hospital staff. Your personal physician can prescribe any necessary services by calling the Outpatient Referral Line at 478-201-6748 (fax: 478-757-0884).
